By submitting the information above, Customer acknowledges that he/she is a duly
authorized representative of the company. By submitting, Customer acknowledges and
agrees that the company is granting (or not) permission to XO to use (or not) company's
name as specified above until otherwise indicated by the Customer. This agreement
may be executed by facsimile and in multiple counterparts, each of which shall be
deemed an original, but all of which together shall constitute one and the same
together. Reference program available to business customers only. Customer must
have had service for at least 6 months.
